Demonstrate skills to develop, implement, and review care and support plans that address complex needs in collaboration with colleagues and wider agencies.

Apply knowledge of sector-specific responsibilities, statutory frameworks, and emerging health models and approaches relevant to their context.
The New Zealand Diploma in Health and Wellbeing (Level 5) Applied Practice is designed to qualify advanced support workers responsible for developing and managing care plans, contributing to continuous improvement, and applying modern health theories and approaches. Learners gain hands-on experience (minimum 200 hours) and demonstrate client-centred competencies tailored to sector specialisation, such as mental health, trauma-informed care, public housing, youth, and more. The programme is not suitable for the aged care sector.

Preferred: Relevant Level 4 qualification OR equivalent sector experience, knowledge, and skill at required level.
